NearFaith Care — Applicant Consent & Verification Authorization

This is the authorization you grant when you submit a Care application — the acknowledgment at the submit step refers to this document. It exists so nothing about verification surprises anyone: you know exactly who we will talk to and what we will say.
1. What you authorize when you submit
By submitting a Care application, you authorize NearFaith to:
- Contact the church you named and disclose to its representative: your name, the fact that you applied for Care assistance, your need category, and the requested amount — so the church can confirm it knows you and that the need appears legitimate. Your written explanation is shared with the church only where needed to evaluate the request.
- Call or text the phone number you provided to confirm it reaches you and to ask follow-up questions about the request.
- Contact the vendor you named (for example the utility company, mechanic, or school) to confirm the amount owed and, if your request is approved, arrange direct payment.
- Screen the application for fraud, including checking it against other Care accounts and requests (for example, duplicate phone numbers or repeated requests), and recording the outcome in an internal audit log.
- Share request details with the administering entity — the participating church or qualified charitable partner that would fund an approved award — to the extent needed for its decision and payment.
2. What you are NOT authorizing
- Publication of your name, situation, or request anywhere public — that never happens.
- A credit check, background check, or any inquiry with a consumer reporting agency — we do not perform these.
- Contact with your employer, landlord, family, or anyone other than the church, the named vendor, and the administering entity.
- Any marketing use of your information, or sale of it to anyone.
3. Your representations
By submitting, you confirm that:
- the information in your application is true and complete;
- you are applying for yourself or your immediate household;
- the church you named genuinely knows you or your household;
- you are a Christian with a genuine ongoing connection to the local Christian church you named, and you understand NearFaith Care is a religious benevolence program of the Christian community with eligibility limited accordingly;
- you understand that assistance is by application only, that every application is manually reviewed and decided at the sole discretion of the appointed NearFaith Care Review Board, and that submission — even with full church verification — does not guarantee assistance and creates no entitlement or contractual right to it.
4. Withdrawing consent
You may withdraw your application, and this authorization, at any time before a decision by replying to any Care email. Verification steps already completed (for example, a note that your church confirmed knowing you) remain in our records under the retention rules in the Care Privacy & Data Retention Notice, but no further contact will be made.
These program documents may be updated from time to time; material changes are posted here with a revised version line. The version in effect when you submit an application governs that application.
